Your first visit will be approximately 1 hour in duration. We ask that you arrive 15 minutes before your appointment time to complete the paperwork.

Please bring with you:

1.         Your driver’s license
2.         Your insurance card(s) – if you wish us to bill insurance
3.         A list of medications/vitamins/supplements you are taking and their dosages.
4.         Your physician’s referral/physical therapy prescription – if you have one
5.         Filled-out paperwork - if you downloaded from our website (below)

Dress comfortably for the visit.

If you need to reschedule or cancel your first appointment, please contact us at least 24 hours before your appointment time.

Paperwork:

If you choose, you may download the paperwork and have it filled out before you arrive for your initial visit. We have provided the paperwork in electronic form.    

You may either print out the paperwork and hand-write
your information OR you may fill-out the paperwork
online and then print it out. If you enter the information
online, you CANNOT SAVE the document for another
time. You must print the document once you have
finished filling it out. Please bring in your paperwork
with you to your first visit.
